Should you require a new or continued sick note please complete an online consultation from 8am this service will be closed once it has reached capacity, using the ‘admin request’ option. Please remember to include the following:
Your Name
DOB
NHS number (if you know it)
Date you want the sick certificate to start from
Period of time, how long for example 14 days
Reason for sick certificate request
Please state if you want a paper copy or texted to you.
Your request will be processed within 5 working days. Please download your sick note via the NHS APP or request the sick note be sent via text.
Self Certification
If you are unwell, you can self certify for any illness for the first 7 days of that illness. Please use this link to download a form.
